REQUIREMENTS:

• Must have minimum 8 ft. plow on your personal truck
• Must be available 24/7
• Must have minimum $500k general liability insurance
• Must have proof of workers compensation for employees
• W-9 shall be provided to the Town of Agawam
• Each driver must submit Legal name, Date of birth and Valid Driver’s license
