Report My Meal

The Agawam Health Department is now using the MA Department of Public Health's system called Report My Meal to collect reports of food-borne illness from the public. If you would like to report food poisoning or illness from a food service establishment in Agawam, please use Report My Meal

Flu Vaccinations  


The Agawam Health Department has available doses of senior High-dose Flu vaccine and regular Flu vaccine. Home visits to vaccinate home-bound seniors are available to be scheduled.

COVID-19 Vaccinations


The Agawam Health Department will NOT be offering COVID-19 Booster Vaccinations.  COVID-19 Booster vaccinations may be available at our local pharmacies by appointment.

The MA Department of Public Health has issued updated guidance for the 2025/2026 respiratory illness season for COVID-19 vaccination. The updated guidance and more information about the COVID-19 Vaccine can be found on the MA DPH website. 


Permits and Licenses


License fees and downloadable applications can be found on the left side of our webpage for your convenience.

August 25, 2023: In accordance with the Town of Agawam Chapter 149 Tobacco Ordinance, we have reached our cap of thirty (30) Tobacco Sales Product Permits and cannot issue any additional permits at this time. Applicants who have already been issued a permit will not be affected. Check back for updates or contact the office. 


  • Sharps Disposal: We offer a free sharps disposal program. For more information visit our Safely Dispose of Sharps page. 
  • Appointments for Flu vaccinations can be made by calling Public Health Nurse at 726-9722.

Duties & Responsibilities

The Health Department is responsible for providing educational and personal services to the residents in order to improve the public health of the community. It is also responsible for the provision of public health services in the Agawam Public Schools via the school nursing program.
