PRESS RELEASE

COVID VACCINATIONS 

The Agawam Health Department will be initiating COVID-19 vaccination clinics for those in Phase 2 (age 75 years and older) beginning on Wednesday, February 17, 2021 for AGAWAM residents only.  Proof of residency will be required at vaccination clinic sites for verification.

Clinics will be held at the Bethany Assembly of God Church, 580 Main Street, Agawam.

DO NOT CALL THE CHURCH, they cannot register you, they are only providing us with a venue to hold our clinic.

 

All registrations will be done online using a computer to enter your information and obtain an appointment time.  Clinics will be ongoing over several weeks.  Do not call the Health Department to register, we are not making appointments.

 

Registration assistance is provided through the Senior Center at 413-726-2840 they have staffing to assist with this process.  

 

Information required when registering is your name, address, phone, email address, and insurance information.  You will be asked to answer a screening questionnaire and then select an available time slot for your vaccination.  You do not need to arrive hours before your scheduled appointment, as you have a reserved vaccination.  Arrival too soon will only cause congestion, confusion, long wait lines.   Clinics run smoothly when you follow the procedures & times outlined.
