The Town of Agawam's Age and Dementia Friendly Working Group, with assistance from the Pioneer Valley Planning Commission, is gathering information from older adults and people who are providing care to older adults to participate in the 2020 Livable Community Survey. The survey results will be used to develop a Community Assessment and Action Plan for making Agawam an Age and Dementia Friendly Community. The survey should take no longer than 20 minutes to complete, and all survey respondents who provide contact information will be included in a drawing for gift cards to Stop & Shop. The survey will be open until December 31st, 2020.
